PATIENT / CLIENT AUDIT(S)

A Scheduled Visit Audit / Physician Rx / Care Plan is inclusive of the following;

• Five (5) days of scheduled visit notes for the same discipline on the same individual patient/client in the same certification period.
• The allotted number of scheduled visit notes for the same discipline per day is one (1) for the same individual patient/client in the same certification period.
• A brief review of the physician order for the certification period for the same discipline for the chosen patient/client for the services/disciplines rx.
• A brief review of the care plan for the certification period for the same discipline for the chosen patient/client for the services/disciplines rx.
• A full written report with suggested corrective action(s).

CONSULTING FEE:

$150.00 PER INDIVIDUAL CHART

• A brief review of the Communication Note(s) for the certification period for the same discipline for the chosen patient/client for the services/disciplines rx.
• A full written report with suggested corrective action(s).

CONSULTING FEE:

$200.00 PER INDIVIDUAL CHART

A Full  Chart Audit is inclusive of the following;

• The Complete HCA / HHA Opening Packet.
• A complete review of scheduled visit notes for the same discipline on the same individual patient/client in the same certification period x two (2) - three (3) years.

Example: Cert. Period is 1/1/2022 -3/1/2022 = 1 year, 1/1/2023 - 3/1/2023 = 2 years, and 1/1/2024 - 2/29/2024 = 3 years OR A RANDOM SIX (6) MONTH AUDIT OF THE SAME CHART IN THE SAME YEAR.

• A complete review of the physician order for the certification period for the same discipline for the chosen patient/client for the services/disciplines rx.
• A detailed review of the care plan for the certification period for the same discipline for the chosen patient/client for the services/disciplines rx.
• A complete review of Supervisory Visits for the certification period for the same discipline for the chosen patient/client for the services/disciplines rx.
• A complete review of the Communication Note(s) for the certification period for the same discipline for the chosen patient/client for the services/disciplines rx.
• A full written report with suggested corrective action(s).
• LABS , ADDITIONAL DISCIPLINES, & ECT. ARE EXTRA.

STARTING AT $350.00 PER INDIVIDUAL CHART

CONSULTATION ASSISTANCE WITH A "COMPLIANT" PATIENT DISCHARGE (HCA/HHA, Hospital, Emergency, & Ect).

A Partial Chart Audit that is inclusive of the following;

• The Complete HCA / HHA Opening Packet.
• Five (5) days of scheduled visit notes for the same discipline on the same individual patient/client in the same certification period.
• The allotted number of scheduled visit notes for the same discipline per day is one (1) for the same individual patient/client in the same certification period.
• A brief review of the physician order for the certification period for the same discipline for the chosen patient/client for the services/disciplines rx.
• A brief review of the care plan for the certification period for the same discipline for the chosen patient/client for the services/disciplines rx.
• A brief review of the Supervisory Visit for the certification period for the same discipline for the chosen patient/client for the services/disciplines rx.
• A brief review of the Communication Note(s) for the certification period for the same discipline for the chosen patient/client for the services/disciplines rx.
• COMPLIANT DISCHARGE PROCEDURE
